Our Team at Attentive Home Care Offers Skilled and Experienced Registered Nurses To Care For Your Loved One While in the Comfort of Their Own Homes.
If your family member requires more than what a home health aide can provide, we can provide a registered nurse to deliver primary or supplemental care at home.
We will match you with a devoted RN who will provide higher-level care that is necessary following surgery or a stroke or while managing late-stage Alzheimer’s, Parkinson’s, or other debilitating diseases.
Our nurses will coordinate with a client’s physician to administer the correct type of in-home care.
Skilled Nursing Services Offered by Attentive Home Care Include but are not limited to:
- A Comprehensive Nursing Assessment. Our registered nurse will visit your home and get to know your loved one’s health history. Our Nurse will then build a care plan for all staff to follow. They will complete vitals and a client checkup. Our comprehensive assessment also includes a complete safety evaluation of the client’s residence. This safety assessment provides suggestions for improvements, such as handrails or a shower chair. Along with a complete medical evaluation, our care plans are comprehensive and include client-specific likes and dislikes, allergies, favorite foods, and hobbies.
- Coordinate care with physicians and collaborate with other healthcare providers. Our nurses will be in direct contact with our client’s primary physician. It is our goal to provide continuity of care. (Link on continuity of care, which opens to Jenn’s explanation of continuity of care).
- Assist patients and caregivers to become independent in care by performing and instructing them about their diseases, medications, symptom management, and wound care.
- We provide full-service medication management. Our registered nurses will personally contact our client’s primary physician to ensure we have the most current medication orders. Our nurses will confirm these orders with the pharmacy of choice and fill your loved one’s medication boxes. They will stay on top of refills, ensuring your loved one does not miss a dose.
- We can also refer clients to private physical therapists, dietitians, and other professionals. Attentive Home Care will work with these partnered groups, assisting with instructed therapy and dietary needs.
