- We make appointments for clients.
- Provide transportation to appointments.
- Our caregivers attend appointments and help relay important information their providers should know.
- Facilitate communication with their loved ones while in an appointment.
- Take notes and communicate important information for ongoing care.
- Ensure that prescriptions are sent to the correct pharmacy and that medications are picked up when ready.
- Reorder prescriptions before they run out.
- Follow up on treatment orders such as Physical or Occupational Therapy.
- Ensure that necessary or ordered medical equipment is delivered and set up.
- Find resources for new conditions the client may develop.
- Ensure that information exchanges between the client’s care team are clearly communicated.
- Keep track of the client’s healthcare calendar.
We often have our first encounter with clients when they have suffered a significant health event. They may be hospitalized or recovering in a Transitional Care Unit. These settings are bustling, and patient demand is high. When it is time for patient discharge from either of these, we ensure the transition is as smooth and comfortable as possible.

We make medical and healthcare appointments and ensure that the client has transportation and care assistance while attending appointments.
